  3. Anti-Citrullinated Peptide Antibodies (ACPA): A Specific Biomarker in Rheumatoid Arthritis

In the evolving field of autoimmune diagnostics, anti-citrullinated peptide antibodies (ACPA) have emerged as one of the most specific and predictive biomarkers for rheumatoid arthritis (RA). While the presence of rheumatoid factor (RF) has long been used as a clinical tool, ACPA provide a higher level of specificity, appear early in disease progression, and are associated with aggressive joint damage.

This blog explores what ACPA are, how they’re detected, and why they’re central to understanding and managing rheumatoid arthritis.

What Are Citrullinated Peptides?

Citrullination is a post-translational modification of the amino acid arginine into citrulline, catalyzed by a family of enzymes known as peptidylarginine deiminases (PADs).

Frontiers | Peptidylarginine Deiminase 2 in Host Immunity: Current Insights and Perspectives

This process:

  • Alters the protein’s structure and charge
  • Creates neo-epitopes that the immune system may not recognize as “self”
  • Can occur during inflammation, cell death, or infection

In susceptible individuals, especially those with genetic risk factors (e.g., HLA-DRB1 “shared epitope”), the immune system may begin producing antibodies against these citrullinated proteins.

ACPA in Rheumatoid Arthritis: Clinical Importance

High Specificity

ACPA have >95% specificity for RA

Present in 60–80% of RA patients

Rarely found in other rheumatic diseases or healthy individuals

Early Detection

ACPA often appear years before the onset of clinical symptoms

Their detection enables preclinical diagnosis or monitoring in high-risk individuals

Disease Prognosis

ACPA-positive RA tends to be more severe and erosive

Linked to:

Increased joint destruction

Systemic inflammation

Higher disease activity scores

How Are ACPA Detected?

The most common assay used is the anti-CCP (cyclic citrullinated peptide) ELISA:

  • Uses synthetically modified cyclic peptides containing citrulline
  • Available in second (CCP2) and third-generation (CCP3) formats

Other emerging technologies include:

  • Multiplex bead-based immunoassays (for simultaneous detection)
  • Microarrays (epitope profiling)
  • Point-of-care diagnostics for rapid screening

Why Do ACPAs Matter Beyond Diagnosis?

Stratification of Patients

  • Helps separate seropositive vs. seronegative RA
  • Guides treatment decisions and clinical trials

Target for Future Therapies

  • Research is ongoing to develop therapies that tolerize the immune system to citrullinated antigens.
  • Blocking PAD enzymes to reduce citrullination is another investigational strategy.

Potential Role in Pathogenesis

ACPA may:

  • Bind to citrullinated proteins in joints
  • Form immune complexes
  • Activate macrophages, complement, and neutrophils
  • Promote synovial inflammation and bone erosion

ACPA in Global Autoimmunity Research

International studies, such as the EIRA (Epidemiological Investigation of Rheumatoid Arthritis) and SERA (Scottish Early RA cohort), have shown the utility of ACPA in:

  • Predicting disease development in at-risk relatives
  • Assessing impact of environmental factors like smoking
  • Linking gut and lung mucosal immunity to RA onset
