The frontier of precision medicine just reached a remarkable milestone—a nine-month-old baby, KJ Muldoon, has been successfully treated with a personalized CRISPR-based therapy to correct a previously fatal metabolic condition. This case marks a pivotal shift in gene editing, transitioning from theoretical promise to real-world human benefit.
CPS1 (carbamoyl phosphate synthetase 1) is a mitochondrial enzyme that initiates the urea cycle by converting ammonia and bicarbonate into carbamoyl phosphate. Loss-of-function mutations in CPS1 disrupt ammonia detoxification, causing hyperammonemia, which can lead to neurotoxicity and death if unmanaged.
Conventional treatments rely on nitrogen scavenger drugs or liver transplantation, both of which have limited long-term efficacy.

The therapeutic strategy applied adenine base editing (ABE), a CRISPR-Cas9–derived technology that allows targeted conversion of an A•T base pair to G•C without double-stranded DNA breaks.
Key design elements included:
Before patient infusion, in vitro testing confirmed:
The patient received multiple systemic infusions of the LNP-packaged base editor. Delivery kinetics were monitored via circulating mRNA quantification and liver function biomarkers. Post-treatment analyses showed:
This case demonstrates:
By integrating advanced base-editing platforms with precision delivery technologies, the CPS1 case represents a technical and clinical breakthrough. It is the first demonstration that customized CRISPR therapies can achieve functional correction in humans, providing a template for addressing a wide spectrum of ultra-rare genetic diseases.
